Promotes a positive physical and psycho social environment for the residents. Essential Functions:: * Plans and directs ongoing care in collaboration with shift supervisor, recognizing individual needs of residents and staff. * Monitors work assignments, provides feedback, evaluates performance, redirects staff, and informs Director of Nursing Services of pertinent problems timely. * Communicates and coaches staff regarding policies, completes quality care audits, and supervises professional staff in the daily deliver of resident care. * Assist with new admissions, transfers, and discharges ensuring related documentation is current, signed, and the record keeping meets the policies and procedures. * Reviews and completes resident records and related documentation in a timely manner, including the completion of nursing assessment for each resident within 14 days of admission. * Plans and conducts resident care conferences and develops interdisciplinary plan of care for each resident. * Regularly observes audits and assesses each resident's condition, status, and care requirements and takes appropriate action to assure care needs are met. * Develops nursing care plans for each resident that meets the needs of the resident and the physician recommendations and reports changes in condition of residents that requires nursing intervention notifying the physician, director of nursing services, and family. * Transcribes and carries out or delegates physician orders in accordance with the nurse practice act. * Documents all pertinent information in the interdisciplinary notes and on the 24-hour DNS report form timely and accurately. * Performs other related duties as assigned. Qualifications:: * Must have a current, unencumbered RN license. * Associate Degree in Nursing or Bachelor of Science in Nursing. * Minimum of 1 year working in geriatric nursing, including experience with medication administration, delivery systems and pharmacies. * One year in supervisory/management position, with experience with budgets, staff development, training and scheduling. * Additional course work in management an asset. * Excellent written and oral communication and motivational skills are essential to success. * Ability to creatively problem solve in both resident care and employee management situations. * Individual must be a team player, well organized, and flexible. : EOE/M/F/VETS/DISABLED

DUTIES AND RESPONSIBILITIES:: Are determined by the center and may include, but are not limited to the following: * Upon admission and periodically (not less than monthly), quantitatively analyze the medical records to assure completeness, accuracy, and internal consistency while the patient is in the center. * Prepare and assemble the medical record on admission, prepare labels for charts and room, complete heading on all forms, stock necessary forms under the appropriate tabs. * Determine upon admission of patients whether additional transfer data is needed and obtain missing information. * Assist the medical staff and allied health personnel in updating the records. * Maintain the flow of reports to the in-house medical records. * Maintain Overflow File by removing outdated forms from the chart holder at the nursing station and filing, in the same chart order, in the overflow charts on a timely basis. * Arrange the medical record in the proper discharge order upon dismissal of the patient and check quantitatively to assure completeness and accuracy within thirty (30) days of the discharge or in accordance with state requirements. * Assist with MOS system as instructed. * Maintain the inventory of medical record supplies at all nurses stations. * Copy records for release of information and log in disclosure log. * Other duties as may be assigned from time to time.

Utilizes the 1-2-3 Admission Protocol to insure admission decisions are made in a timely manner. Communicates promptly with referral sources regarding admission decision. Communicates special needs for admission to appropriate staff Â§ Inputs daily referral sources and admissions data into the automated referral system.: Follows up daily on all active and pending inquiries. Ensures that every admission is educated about and encouraged to sign up for the Alternative Dispute Resolution program, in states where the Company chooses to offer it. Coordinates the development and implementation of admission plans; serves as a chairperson for weekly Census Development meetings, monitors budgets and tracks results. Maintains a working knowledge of Federal and State regulations and reimbursement guidelines (Medicare and Medicaid). Assists with managed care referral process. May provide assistance with new employee orientation for the admission process. Maintains a database of medical contacts/community resources along with knowledge of industry trends and legislative/regulatory issues and communicates this information to facility staff and community contacts.
